Dr. Yvonne Prince has been located on the SMDH campus since 1994. Dr. Bob Carnett returned to Salem in January 2015. Dr. Julius Punzalan and Dr. Sarfaraz Jasdanwala joined us in July.
We have exciting news to share about our SMDH Physician Clinic and many changes happening in 2015.
SAME DAY appointments are now being offered and extended hours for the clinic are from 8am-8pm Mon-Thur and Friday from 8am-6pm. Call to take advantage of this new service at 573-729-8000. Walk-ins are also welcome now!
September: Effective October 1st, Craig Tatom, APN-C, moves to the Physician Office Building 3, so all SMDH physicians and staff are housed in the same building to better facilitate patient care in the primary care arena.
July: Please welcome our newest SMDH physician, Dr. Sarfaraz Jasdanwala, who started seeing patients on July 15th.
April: Salem Memorial District Hospital Board of Directors voted to contract with Julius Punzalan, M.D. to join SMDH Family Medicine clinic effective immediately and moved to the new clinic location in Physician Office Building #3 on July 13th. Dr. Punzalan has joined Dr. Yvonne Prince, Dr. Bob Carnett, Dr. Vicky Gulley, Dr. Sarfaraz Jasdanwala, as well as Craig Tatom, FNP, Sandra Headrick, FNP, and Nathan Miracle, PA-C. Dr. Punzalan has been an independent physician member of SMDH Medical Staff the past 13 years. He comes to SMDH Family Medicine wanting to concentrate more on office and hospital clinical medicine, while leaving the business side to the hospital.
March: Dr. Prince's clinic moved into Physician's Office Building #3 on March 2nd, and is now working under the clinic name SMDH Family Medicine. Nathan Miracle, PA-C, joined the clinic.
February: We are pleased to announce Sandra Headrick, FNP, returned to work with Dr. Yvonne Prince.
January: Read some history of our physician changes, including Dr. Bob Carnett, who returned to work in the Salem community.
